The agentic AI conversation often starts optimistically enough. AI agents, whether custom-designed or product copilots, are transforming productivity as agentic workflows unlock operational efficiency. The ROI narratives are compelling, and the vendor demonstrations certainly are polished. Then someone asks, “Who’s actually managing all of this?”
That question separates CXOs who will navigate the AI era cleanly from those who will spend the next several years unwinding damage they didn’t see coming. Based on conversations with technology strategists and enterprise architects, the governance answer at most organizations right now is: nobody.
The Governance Mirage
The current generation of AI copilots and agents operates on a principle that sounds reassuring. As these tools move fluidly across email, CRM, SaaS platforms and more through a single interface, they act with the persona and permissions of the user(s) they serve.
“You’re not asking it to do things that you don’t have access to,” explains Tim Crawford, CIO strategic advisor at AVOA. “It makes the governance conversation very easy. The agent assumes the persona of Tim or George and uses those credentials to access those other systems. Whatever one has access to, the access and governance models go with the agent,” he says.
“However, some of these agents are going to be highly autonomous and do things with your data. What’s the risk of those agents doing something unintended? It’s huge,” Crawford says.
In practice, that transfer of access rights means the agents are inheriting deterministic access rights and then making decisions the user never reviewed, never approved, and may never know occurred. Permission inheritance answers the question of what an agent can reach. It doesn’t necessarily answer the question of what it will do when it gets there. That gap is a source of significant enterprise risk.
Mature enterprises have replaced the simplistic “act as the user” inheritance model with a structured agent-first governance framework. They provision every agent with its own dedicated identity, scoped just-in-time permissions, and explicit delegation tokens rather than full impersonation of someone’s permissions. This way, routine actions run autonomously; high-risk ones trigger mandatory human-in-the-loop approval. And all activity is monitored in real time, with behavioral guardrails and immutable audit trails that capture prompts, reasoning chains, and outcomes. A centralized agent registry enforces ownership, purpose limits, and periodic reviews.
This framework, identity controls, and runtime observability close the gap between what an agent can reach and what it can do.
The AI Sprawl Few Have Planned For
Another governance risk is CXOs managing environments where spinning up an AI agent is as easy as building a spreadsheet report, and where the proliferation of semi-autonomous processes that consume compute, touch-sensitive data, and execute consequential workflows flourishes largely outside formal governance structures.
Unlike forgotten reports, agents don’t sit idle. They run. They run around the clock, potentially even after their creators change roles and after the business context that justified them has shifted. The organizations that have focused heavily on removing frictions from agent deployment — and most of them are, right now — are creating a future governance crisis for themselves. “Once you start to remove some of those friction points, and you start to allow people to build out these agentic workflows, you’ll start to see agentic sprawl. Everybody has a litany of agents that are out there, and many are unknown,” adds Crawford.
The imperative for AI governance is pressing for CXOs, and the governance conversation can no longer be deferred to a future phase or federated entirely to departmental teams.
CXOs must now be able to answer the question: Who owns agent lifecycle management? How is not only access, but authorization being governed across systems of record when a single AI cuts across all of them? And when something goes wrong, who is accountable?
The organizations tackling those questions today will be in a position to answer them later. Those waiting for the dam to break will find, as they often do, that it breaks during a moment of crisis.
